IVF : Helping You to Become Parents

IVF (In vitro Fertilization) is the Famous Procedure for Infertility. IVF treatment blessed many couples with babies. IVF is done with women own eggs/donor eggs and sperm/donor sperm. In vitro fertilization mainly helps to treat damaged and blocked fallopian tubes, endometriosis, cervical issues, male and unexplained infertility problems.

It is the process of combining eggs with sperms in a Petri dish in a sterile place, laboratory. Once the fertilization is over, the embryos are transferred into women uterus and the pregnancy is carried out.

IVF is recommended when the following condition exists
  • Diminished Ovarian Function or fallopian tube damage
  • Endometriosis
  • Anovulation and Polycystic Ovarian Syndrome
  • Unexplained infertility
  • Age Related Infertility Problems
  • Those who are at risk of genetic disease such as cystic fibrosis
The IVF cycle has four standards.

Ovarian stimulation: For producing multiple eggs hyperstimulation drugs such as Clomiphene is prescribed. Human Chorionic gonadotropin is given to nourish the egg maturation after 8-14 days.

Egg retrieval: After the hCG injection the eggs are retrieved within 34-36 hours. The OBG uses an ultrasound guided probe into the vagina for egg retrieval. With the help of a thin needle, the liquid is drained from the follicles.

Fertilization and Embryo Culture: The eggs are evaluated for its quality and maturity. Selected eggs are placed in the laboratory and then transferred into an incubator. The sperms and eggs are either place together for infusion or single sperm is injected into the egg. Thus insemination is performed.

Embryo Transfer and Cryopreservation: After the egg retrieval one or more embryos are implanted in the uterus. If the embryos are excess those will be frozen for future use. About two weeks is required to determine whether the pregnancy is achieved.

GIFT (gamete intrafallopian transfer) and zygote intrafallopian transfer (ZIFT) are other two procedures of IVF. GIFT is a method of egg harvesting as with IVF and mixing with sperm, then injected into the fallopian tube where the fertilization occurs. In ZIFT, before transferring the eggs into the fallopian tube, the egg is fertilized with sperm in the laboratory. Women who at least have one functioning of fallopian tube can undergo this procedure.

Delay in pregnancy due to Fibroids ? Laparoscopy is the right option

Fibroids also called uterine myomas, leiomyomas or fibromas are small rigid  tumors appear in  the walls of the reproductive system of a women ( uterus). Women in the age group of 20 to 50 have more chances of affecting this condition. Most of the fibroids are harm less in nature.

uterine-fibroids-1024x728.jpg

How fibroids causes infertility ?
  • While growing fibroid change the shape of cervix preventing the entry of sperm into uterus
  • It restricts the momentum of sperm or embryo
  • It will block the fallopian tubes
  • Badly impact the lining size of uterine cavity
  • It reduces the blood flow to the uterine cavity preventing embryo from sticking to the uterine wall thus affecting its development

Advancement of technology in the medical field has resulted in minimally invasive technologies that reduce the pain as well as recovery time of the patient. Nowadays Laparoscopic Surgery is effectively used for removing fibroids which causes infertility in women. It is also called Laparoscopic Myomectomy. In this doctor will make a small opening near the belly button. Then laparoscope, a slender tube fitted with camera is inserted into abdomen. Doctor will operate in real time by looking into monitor and inserting smaller instruments through it.

Infertility and its Treatment – Understand it better

Infertility is defined as a couple’s inability to conceive after one year of unprotected intercourse. Infertility is perceived as a social problem in our country. However, advances in medical science have made it possible for infertile couples to experience the joys of parenthood.

What are the main treatments used for infertility?

Fertility treatments can be grouped into three categories:

Medicines to improve fertility:

  • Clomiphene is a medicine, which has been used to help with fertility for many years. It’s taken as a tablet. It releases more gonadotrophin hormones than normal. The extra amount of gonadotrophin hormones may stimulate the ovaries to ovulate.
  • Medicines that contain gonadotrophins are another type of treatment.  If this is injected, these medicines can also improve the fertility in men with certain types of hormonal problems that can affect the sperm count.
  • Metformin may also help to improve the fertility in some women with PCOS. It is commonly used to treat people suffering from diabetes.

What are the surgical treatments for fertility?

There are different types of surgical treatments for the fertility. Here are some of them:

Fallopian tube problems:  This surgery may help some women with infertility caused by fallopian tube problems.
Endometriosis:  This is a condition that occurs when cells from the lining of the womb (uterus) grow in other areas of the body. This surgery may help to improve fertility in women with endometriosis.
Polycystic ovary syndrome (PCOS):  An operation on the ovaries may be suitable for some women with PCOS.
Fibroids:  For women with fibroids, surgery (to remove the fibroid) may be considered if there are no other explanations for the infertility.

What is assisted conception? What are the processes associated with them?
Assisted reproductive technology (ART) is one technology, which is used to achieve pregnancy in various procedures such as fertility medication, artificial insemination, in vitro fertilization (IVF) and surrogacy. It is a reproductive technology used primarily for infertility treatments, and is also known as Fertility Treatment. Here are the various procedures associated with them:
Intrauterine insemination (IUI)
This is the process by which sperm are placed into the woman's womb (uterus). It is done by using a fine plastic tube which is passed through the neck of the womb (the cervix) into the womb. Sperms are passed through the tube. It is relatively a straightforward procedure. It can be timed to coincide with ovulation (about halfway through a monthly cycle) in women who are still ovulating. Fertility medicines may also be given beforehand, to maximize the chance of ovulation occurring. Women, who have this procedure, need to have healthy fallopian tubes to allow the egg to travel from the ovary into the womb. If successful, fertilization takes place within the womb. Then the sperm can be used either from the male partner, or from a donor. If IUI does not work, couples tend to move on to try other methods described below.
In vitro fertilization (IVF)
IVF means In vitro fertilization outside the body. In vitro literally means in glass (either, in a laboratory dish or test tube). IVF is used in couples, whose infertility is caused by blocked fallopian tubes, or where their infertility is unexplained. It may also be used where there are certain problems with ovulation or a combination of factors causing infertility.
Gamete intra-fallopian transfer (GIFT)
A gamete is an egg or sperm. Eggs and sperm are collected in the same way as for IVF. The eggs are mixed with the sperm. The mixture of eggs and sperm is then placed into one of the woman's fallopian tubes. Therefore, unlike IVF, the sperm fertilises the egg naturally inside the woman's fallopian tube or womb, and not outside the body in a laboratory dish. GIFT is no longer a recommended process to be used.
Intracytoplasmic sperm injection (ICSI)
This technique involves an individual sperm being injected directly into an egg. Some cases of infertility are due to the sperm of a male partner, not being able to penetrate the outer part of the egg to fertilise the egg. ICSI can also be used when a male partner has a low sperm count, as only one sperm is needed.
Egg donation
This involves stimulating the ovaries of a female donor with fertility medicines, and collecting the eggs which form. The eggs are mixed with and fertilised by sperm of the recipient's partner (similar to IVF). After 2-3 days, embryos are placed in the womb of the recipient, via the cervix.
Embryo donation
Couples who have had successful IVF treatment may decide to donate any spare embryos to help other infertile couples.

What is surrogacy?

Surrogacy is a procedure in which a woman acts as a temporary mother by allowing her womb (uterus) to carry a pregnancy for another couple.

When is the help of surrogate mothers required?

  Absence of uterus from birth.

  • Uterus has been accidentally removed at a young age, either because of heavy bleeding or an accident during the first childbirth, or due to a tumor.
  • Repeated IVF failure. When failure is suspected to be due to a defective or diseased uterus, as in endometriosis or adhesion (gluing) between two inner walls of the uterus.
  • Sometimes, working women want surrogacy in order to avoid absence from work. Alternatively, some married women prefer surrogacy in order to not lose their abdominal shape or figure. However, these indications have not been accepted in the ‘guidelines’ being prepared for the ART clinics in India.

Chocolate Cyst Removal

Chocolate Cysts are a type of ovarian cyst which is also known as endometriosis. These are cysts in ovaries occur by endometrial tissue that is similar to the lining of the uterus. The word "Chocolate cyst” is derived from the physical appearance of the cyst. The old dried blood makes the color of the cyst as brown. It can vary its size from 0.5 to 8 inches in diameter. if the cyst breaks, contents in it will enter into the pelvic cavity and ovaries. This causes pelvic pain and some of the pelvic organs may stick together. If it happens with ovaries and fallopian tubes it can result in infertility. If the chocolate cyst is affecting women during their reproductive period it may cause pelvic pain associated with menstrual cycle.

IVF- Directs You To A Beautiful Hope

In-vitro fertilization is the fertility treatment that is performed in the laboratory. It uses special medical techniques. In IVF, men’s sperm and woman’s egg are fertilized in laboratory. The main aim of this treatment is to treat women with damaged fallopian tubes. It is the common reproductive technology used. Recently this treatment is used as fertility treatment for women with endometriosis, unexplained infertility or in male fertility problems.

IVF is prefered, if you and your partner are diagnosed for factors which include Low sperm count, antibody problems that harm sperms or eggs and problems with ovulation. In vitro fertilization has 4 stages :
  1. Ovarian stimulation : Ovarian stimulation drugs is used for stimulating the ovaries to produce multiple eggs. After 8-14 days, another drug is used for nourishing egg maturation.
  2. Egg retrieval : For retrieving the eggs, ultrasound-guided probe is inserted to the vagina by the doctor. A needle is then used to suck liquid from a small gland (follicles) and lot of eggs are retrieved.
  3. Fertilization and embryo culture : In this stage, maturity and quality of the eggs are evaluated. Eggs which are selected after evaluation are transferred into a closed apparatus in which small babies are placed. Then inseminated with sperm, either by injecting a single sperm into the egg or by placing sperm together with the egg.
  4. One or more embryos are implanted in the uterus following egg retrieval (after 1 - 6 days). Excess embryos may be frozen and saved for future use. It takes about 2 weeks to determine if pregnancy has been achieved.
It is not necessary that all IVF cycles should result in pregnancy and then in live births. Certain variables which may even include the age of patients makes the success rates of IVF highly depended. Some women are even subjected to acupuncture which may increase the chance for pregnancy.

IVF - A New Ray of Hope!!!!!

Fertilization and pregnancy are complex procedures that depend upon various factors, including:  the Production of healthy sperm,  healthy eggs, unblocked Fallopian tubes that permit the sperm to fertilize with the egg, the sperm's ability to fertilize the egg, the capacity of the treated egg to get embedded in the woman’s uterus, and sufficient embryo quality. If one of these components is impeded, it can bring about Infertility.

IVF Treatment in Madurai

It is a common assumption that infertility is mainly related to women. But the fact is that male and female have an equal chance for infertility. There are no signs or symptoms associated with an infertility. Understanding your body and getting regular checkups will help you to distinguish the issue.

Advancement in the medical field offers number of treatment options for couples who are trying to conceive a child. Hormonal treatments, ovulation induction, intrauterine insemination and more advanced treatments like In Vitro Fertilization (IVF), ICSI, egg/ sperm donation and even embryo donation are available.

IVF is a procedure in which fertilization is performed manually by combining egg and sperm in a laboratory dish, and place the fertilized embryo in the uterus. IVF can be performed with own eggs and sperms or with donors eggs and sperms.
IVF can be used as an option if you and your partner have been diagnosed with
  • Low sperm count
  • Problems with the uterus or fallopian tubes
  • Endometriosis
  • Antibody problems that harm sperms or eggs
  • Problems with ovulation
  • An unexplained fertility problem

How Does IVF Works

Eggs are recovered by ultrasound guided needle aspiration under light sedation. This includes no surgical cut, and no postoperative pain. You simply leave the hospital straightforwardly from the working room, with no pain, and return three days after to have the embryo (or fetuses) very simply into the uterus through the cervix with a tiny catheter. No incision and anaesthesia are required. After a hour you are able to go home.

IVF Treatments in Madurai

In order to recover these multiple eggs for IVF, the patient must undergo injections with hormones and watchful observing of her ovaries by ultrasound and her hormone levels by blood tests each day or every other day until she is prepared for the egg retrieval.

ART: Assisted Reproductive Technology is the application of clinical or laboratory technology used to  achieve pregnancy by procedures such as in vitro fertilization, surrogacy, artificial insemination etc. ART is generally invasive and expensive. In this technique female's egg is made to fertilize with males sperm in a laboratory and embryos are transferred to the uterus after three to five days of fertilization.
1. IUI (Intrauterine Insemination): It is less expensive, less complex and tried in patients with uncomplicated cases of fertility. Here the sperm from the partner is washed and placed into the uterus near to the egg, at the time of ovulation.

2. IVF (In Vitro Fertilization): It is the process of combining females egg with males sperm in a laboratory dish and transferring the embryo to the uterus.
It is expensive and recommended when both the fallopian tubes are blocked. IVF requires frequent blood tests and daily hormone injections.
3. ICSI (Intra Cytoplasmic Sperm Injection): It is the process in which a single sperm is injected into an egg which is retrieved from IVF. It is usually done when male infertility is the main reason for infertility.
